ABEM/AOBEM Board Certificate or Board eligibility letter Clean file/background with no malpractice Procedure logs (last 2 years) Updated CV, drivers license, color photo, state medical license (including licensure held in other states), CSR, DEA, education certificates, 5 year malpractice history, need narrative & legal documentation if provider has claims, ECFMG if applicable, ABEM/AOBEM Board Certificate, Board eligibility letter, TB/PPD/Quantiferon/Chest x-ray (within 12 months), COVID, and FLU (seasonal).
